L’Ago della Bilancia è un testo sul tema dell’obesità ideato con l’intento di mettere in connessione la realtà clinica, la medicina narrativa e l’approccio scientifico.

Il tema dell’obesità è affrontato in nove capitoli, ognuno dei quali si concentra su un aspetto specifico della cura della persona con obesità.

L’Ago della Bilancia accompagna il lettore iniziando dalla richiesta di aiuto da parte della persona con obesità, prosegue prendendo in esame le strategie terapeutiche attualmente disponibili, e si conclude esplorando gli effetti della pandemia da SARS-CoV-2.

Ogni capitolo del testo è suddiviso in tre sezioni:

• Riflessioni dalla pratica clinica;

• Storie narrate di persone con obesità;

• Evidenza scientifica.

Ogni capitolo fornisce, inoltre, una scheda pratica con strumenti utili e applicabili nelle attività cliniche quotidiane dei professionisti sanitari. Nel testo si è scelto di utilizzare il linguaggio “people-first”, cioè un tipo di linguaggio che riconosce prima la persona rispetto alla condizione da cui è affetta, per promuoverne l’uso tra gli operatori sanitari.

L’Ago della Bilancia è un testo che, come tutti noi, si mantiene in equilibrio tra la pratica clinica, il vissuto della persona con obesità e l’evidenza scientifica.

Nutritional Psychology

In today's world, the food we choose must, on the one hand, satisfy emotions, senses, pleasures, and serve a socializing function. On the other, it must reflect the principles of the "diet culture" in which we are immersed, a culture that pervades us with images, information, and standards to be respected. A culture that puts us in real time in contact with a multitude of different figures, from professionals to influencers, who inundate us with information, sometimes even conflicting.
Understanding eating behavior from a nutritional and psychological perspective means moving toward a broader perspective, aimed at identifying the factors that can somehow predict eating behavior.

Considering an approach that involves the use of multilevel strategies—psychological, social, environmental, and biological—is therefore certainly the first step to truly triggering a change in eating behavior that sees people actively involved in choosing their own habits.

The text, divided into 7 chapters, addresses the main topics related to nutrition: from eating disorders to the theme of aging, including anxiety, stress, obesity, and mood disorders.

Chapter 14 - Obesity, Adipokines and Thyroid Dysfunction

Obesity is characterized by an increase in body fat mass stores. Adipose tissue produces a variety of adipokines, which exert local and systemic effects and regulate several tissues and organs.  Thyroid dysfunctions are classified as secondary causes of obesity, and the effects of thyroid hormones on growth, thermogenesis and metabolism have been clearly elucidated. A link between thyroid and obesity has been identified, showing that thyroid dysfunction can cause metabolic alterations and obesity. On the other hand, only recently a lively interest in exploring the possible effects of adipokines on thyroid axis has started. Adipokines have been identified as possible causative or protective factors in the development of thyroid dysfunction in humans. Clinical studies report controversial results. Aim of this chapter is to present and explore available data on adipokines and thyroid dysfunction. This chapter focuses on leptin, adiponectin, resistin, vaspin and visfatin concentrations in the presence of thyroiditis, hypothyroidism and hyperthyroidism in humans.
